Agent change application send but not changed
Dear sir, policy no. 0631002817p112146119 (medical health policy) dt. 04/12/2017. We gave an application to change our insurance agent but UIICL has renewed our policy to the old agent. I want a new agent with code - AGD0019996. My contact no is 9879041417.

But till date I have not received any renewd policy nor any mediclaim health card. Above policy was purchased in 2009 and this has been continued with no claim till date. This was attached to DHS ans last year they were unable to renew it and I happen to go to Mumbai and handed over premium cheque to DHS OFFICE and it was promised that the policy will be renewed and and sent to my address but till date it has not been received. The same be sent immediately as the cheque has already incashed and also inform me as to who has been looking after this policy.

In june 22 i submit the all my documents of health insurance my wife smt.nitika kaura in 9 july i received a sms that your clame done update your bank i do that till today i not any kind of action from your side 10 to15 times i go to you office but the results zero 100 times i connected with my agent mr.shaminder agent code AGD036303 he cannot pick my phone very very bad service form my agent. I also meet with the head of Ludhiana manager mr. umang but result is zero. my agent can't do his job properly. please release my claim. my connect number is 9872624062.

Yet to receive the reimbursement of OPD claims in r/o SAIL Group
Policy No. 041100/28/18/P102461147, Claim No. MDI4989016, MDI4989003 & MDI5003312
I am holding the SAIL Group Medical Policy as indicated above. It has been a very long time that despite submission of all documents I have not yet received the OPD reimbursement against the above said three claims. Kindly note that all the premiums including the renewal premium have been paid in time and as per the circular issued by SAIL I am yet to receive all the claims raised by me as per policy terms and conditions against the OPD investigation/ treatment done till 10.07.2019 which was also the directive issued by Hon'ble High Court Delhi. Please arrange to reimburse the said claims at an early date.
